Serious ethical violations in medicine, such as sexual abuse, criminal prescribing of opioids, and unnecessary surgeries, directly harm patients and undermine trust in the profession of medicine. We review the literature on violations in medicine and present an analysis of 280 cases. Nearly all cases involved repeated instances of intentional wrongdoing, by males in nonacademic medical settings, with oversight problems and a selfish motive such as financial gain or sex. More than half of cases involved a wrongdoer with a suspected (...) personality disorder or substance use disorder. Despite clear patterns, no factors provide readily observable red flags, making prevention difficult. Early identification and intervention in cases requires significant policy shifts that prioritize the safety of patients over physician interests in privacy, fair processes, and proportionate disciplinary actions. We explore a series of 10 questions regarding policy, oversight, discipline, and education options. Satisfactory answers to these questions will require input from diverse stakeholders to help society negotiate effective and ethically balanced solutions. (shrink)

C.C.E. Schmid’s doctrine of intelligible fatalism was immensely influential in the immediate reception of Kant’s philosophy. Existing treatments of this doctrine, largely neglected by modern scholarship, echo uncharitable interpretations espoused by Schmid’s contemporaries. I demonstrate that Schmid’s intelligible fatalism is more coherent and philosophically robust than hitherto recognized. I argue for a novel interpretation of Schmid’s account of rational agency, showing that intelligible fatalism is compatible with his conceptions of freedom, obligation, and imputation. Specifically, I argue that the role of (...) consciousness in this account carves out conceptual space for a distinction between the theoretical and the practical that is sufficient to render intelligible fatalism consistent with these conceptions. We report the identification of a recurrent, 520-kb 16p12.1 microdeletion associated with childhood developmental delay. The microdeletion was detected in 20 of 11,873 cases compared with 2 of 8,540 controls and replicated in a second series of 22 of 9,254 cases compared with 6 of 6,299 controls. Most deletions were inherited, with carrier parents likely to manifest neuropsychiatric phenotypes compared to non-carrier parents. Probands were more likely to carry an additional large copy-number variant when compared to matched controls. The clinical (...) features of individuals with two mutations were distinct from and/or more severe than those of individuals carrying only the co-occurring mutation. Our data support a two-hit model in which the 16p12.1 microdeletion both predisposes to neuropsychiatric phenotypes as a single event and exacerbates neurodevelopmental phenotypes in association with other large deletions or duplications. Analysis of other microdeletions with variable expressivity indicates that this two-hit model might be more generally applicable to neuropsychiatric disease. © 2010 Nature America, Inc. Over a long career of teaching and writing in the area of moral theology Charles E. Curran has experienced large areas of agreement with John Paul II on issues of social justice even while in other areas of personal and sexual issues the two are in serious disagreement. This phenomenon of agreement/disagreement has suggested to Curran that the pope is guilty of using a double methodology in his moral theological writing. Curran's book, The Moral Theology of Pope John (...) Paul II, seeks to uncover and substantiate the root of their agreements and disagreements. This article seeks to evaluate Curran's theory. This analysis is done in two parts: first, an examination of the evidence that Curran presents to support his charge against the pope, and second, an examination of the alternative possibility that it is Curran who has the double methodology rather than the pope. In a letter to William Molyneux John Locke states that in reviewing his chapter 'Of Power' for the second edition of An Essay Concerning Human Understanding he noticed that he had made one mistake which, now corrected, has put him "into a new view of things" which will clarify his account of human freedom. Locke says the mistake was putting “things for actions” on p.123 of the first edition, a page on which the word 'things' does not appear (The (...) Correspondence of John Locke. E.S. de Beer, ed. In this working paper, we discuss why researchers and policymakers need to better understand how different theoretical accounts of stability lead to different frameworks of analysis. Though ubiquitously mentioned, stability remains an under-theorized notion in security studies. Expert accounts tend to present stability as a generic description, readily applicable to most political phenomena Ð a stabilized state, a stable region, an unstable society. While seemingly equivocal, such uses exhibit different conceptualizations, exhibiting different features and demanding multiple research programs. To date, (...) most accounts of stability tend to take stability to be an (unconstructed) natural phenomenon or a non-derivative social construction. In this introductory paper, we claim that there is a third way to account for stability that remains unexplored in security studies. Simply put, it involves taking stability to be a derivative social construction (i.e., systemic fallout). We then argue that this alternative account should serve as the theoretical basis for an independent research program within stability studies. (shrink)

This paper goes beyond — transcends — “pedagogy as justice,” recognizing that justice, particularly in these present times, may not be enough. Its wager is with pedagogies of and for life; pedagogies that plant and cultivate, that push and enable other modes of living, despite the capitalist-modern-colonial-racist system, beyond the system, and in the system's margins, borders, fissures, and cracks. These pedagogies, as Catherine Walsh argues here, are necessarily tied to and constitutive of decolonial(izing) praxis, a praxis that, while (...) not negating justice, takes us beyond it. The argument here is for the pedagogical-praxistical work being done now, and yet to be done; the actional thought, thoughtful action, and ongoing creation and construction of an otherwise for existence, for dignity, and for life in these times of multiple entwined pandemic-viruses, including capitalist greed, systemic racism, heteropatriarchy, and existential-territorial dispossession in which COVID is/was one more thread of the entwine. People don’t just compete, however; they also cooperate. John Roemer argues that attempts by orthodox game theorists to account for cooperation leave much to be desired. Unlike competing players, cooperating players take those actions that they would like others to take—which Roemer (...) calls “Kantian optimization.” Through rigorous reasoning and modeling, Roemer demonstrates a simpler theory of cooperative behavior than the standard model provides. This article summarizes a variety of current as well as previous research in support of a new theory of consciousness. Evidence has been steadily accumulating that information about a stimulus complex is distributed to many neuronal populations dispersed throughout the brain and is represented by the departure from randomness of the temporal pattern of neural discharges within these large ensembles. Zero phase lag synchronization occurs between discharges of neurons in different brain regions and is enhanced by presentation of stimuli. This (...) evidence further suggests that spatiotemporal patterns of coherence, which have been identified by spatial principal component analysis, may encode a multidimensional representation of a present or past event. How such distributed information is integrated into a holistic percept constitutes the binding problem. How a percept defined by a spatial distribution of nonrandomness can be subjectively experienced constitutes the problem of consciousness. Explanations based on a discrete connectionistic network cannot be reconciled with the relevant facts. Evidence is presented herein of invariant features of brain electrical activity found to change reversibly with loss and return of consciousness in a study of 176 patients anesthetized during surgical procedures. A review of relevant research areas, as well as the anesthesia data, leads to a postulation that consciousness is a property of quantumlike processes, within a brain field resonating within a core of structures, which may be the neural substrate of consciousness. This core includes regions of the prefrontal cortex, the frontal cortex, the pre- and paracentral cortex, thalamus, limbic system, and basal ganglia. (shrink)

When faced with adverse circumstances, there may be a tendency for individuals, agencies, and governments to search for a target to assign blame. Our focus will be on the novel coronavirus outbreak, where racial groups, political parties, countries, and minorities have been blamed for spreading, producing or creating the virus. Blame—here defined as attributing causality, responsibility, intent, or foresight to someone/something for a fault or wrong—has already begun to damage modern society and medical practice in the context of the COVID-19 (...) outbreak. Evidence from past and current pandemics suggest that this tendency to seek blame affects international relations, promotes unwarranted devaluation of health professionals, and prompts a spike of racism and discrimination. By drawing on social and cognitive psychology theories, we provide a framework that helps to understand the effect of blame in pandemics, when people blame, whom they blame, and how blame detrimentally affects the COVID-19 response. Ultimately, we provide a path to inform health messaging to reduce blaming tendencies, based on social psychological principles for health communication. (shrink)

The ease with which genotyping technologies generate tremendous amounts of data on research participants has been well chronicled, a feat that continues to become both faster and cheaper to perform. In parallel to these advances come additional ethical considerations and debates, one of which centers on providing individual research results and incidental findings back to research participants taking part in genetic research efforts. In 2006 the Industry Pharmacogenomics Working Group offered some ‘Points-to-Consider’ on this topic within the context of the (...) drug development process from those who are affiliated to pharmaceutical companies. Today many of these points remain applicable to the discussion but will be expanded upon in this updated viewpoint from the I-PWG. The exploratory nature of pharmacogenomic work in the pharmaceutical industry is discussed to provide context for why these results typically are not best suited for return. On campuses across the country in recent years, speech codes have been introduced, the “re-education” of professors (...) urged, visiting speakers on controversial subjects silenced, and classes monitored by administrators eager to do the bidding of various special interest groups. As universities have come more and more to see their role as that of some sort of multipurpose social project, education itself has become less pressing a priority. Thus, while money is found to hire sexual harassment officers, employment equity advisors and “wellness” mavens, academic posts disappear. Those that remain are frequently filled in accordance with crass gender and race-based quotas, and when that fails to guarantee positions for members of bureaucratically favored groups, contrived “schools” and pseudodisciplines proliferate. Many of these developments, however, are simply the manifestation of trickle-down deconstruction, trickle-down Rortyism, trickle-down “politics of difference,” of the sort that Charles Taylor is prone to rhapsodize. Notions of value, nature and merit, of equality, universality and objectivity are dismissed as mere inventions, “imperial” notions designed to favor the powerful and oppress the disenfranchised. The art [End Page 492] of spoken and written argument, with their ideals of detachment and discipline, are taken as evidence of a smug “androcentrism,” while rationality itself is derided as a “regime” that works “to maintain patriarchal control of knowledge and knowledge production.” Even the notion of evaluating students by means of essays and exams is ridiculed as scandalously “non-feminist.” While past attacks on academic independence have tended to come from outside the university, Canadian academics over the past decade have slavishly accepted their own domestication. More disconcerting still has been the role of pliant university presidents, most of whom have proved eager to unburden themselves of the inconvenience of scholarship and to indulge the cosmological musings of the radical left.Beyond Political Correctness: Toward the Inclusive University bristles both with resentment that such criticisms should even be aired and paranoia about their motivation. According to its contributors, the challenge to political correctness—the very use of the term as an epithet—is the result of a “neo-conservative” attack that has been orchestrated in and “imported” wholesale from the United States (to morally smug Canadians, there can be no more dastardly provenance) and insinuated into public debate by a servile media. The first two articles in the collection, by Dorothy Smith and Lorna Weir respectively, pursue this very theme. Each betrays that dreary habit, particularly prevalent among social scientists, of rendering imposingly technical the most bracingly trite. For instance, Smith writes: “The textuality of public text-mediated discourse is essential to its peculiar temporal and spatial properties of ubiquity and constancy of replication across multiple and various actual situations of watching, reading, or hearing” (p. 25). Translation? The more one sees, reads or hears a view expressed, the more one tends to believe it’s true. Her concern here, of course, is to establish that whoever controls the means of public debate controls its terms as well, that those means happen to be controlled by neo-conservative interests bent on retarding social progress, and that growing public disquiet about educational reform simply confirms the success of that neo-conservative strategy. But the failure of Smith’s account goes far beyond the thoroughly rebarbative nature of her expression.According to Smith, the very term, “political correctness,” is shorthand for an “ideological code” that has been expressly designed by “neo-conservatives” to caricature the good works of the visionary left. An ideological code, she says, operates as a “free-floating form of control,” which, once established, is “self-reproducing” (p. 27).
